The linked form would be used by anyone who is not on the Permanent Absent Voter Ballot Application list for their township. This form, when completed should be submitted to your city or township clerk. (For assistance in obtaining the address of your city or township clerk, see  Michigan.gov/Vote You must request an Absent Voter Ballot by mailing the Absent Voter Ballot Application to your local clerk’s office.  Requests to have an Absent Voter Ballot mailed to you must be received by your clerk no later than  4 p.m. the Friday before the election.

Note the form has check boxes for the Presidential Primary Election and the November General Election but not the August Primary Election. Another form will be mailed to voters on the Permanent Absent Voter Ballot Application list before the August Primary for that election and the subsequent General Election.
